Liver conditions, including liver failure, can occur in previously well infants, children, or adolescents and can be caused by infectious or metabolic causes and reactions to medication.

The functional development of the liver that occurs in concert with growth requires a complicated orchestration of changes in hepatic enzymes and metabolic pathways. After 2 years of age, the infant has a fully mature liver capable of performing all its function at levels comparable to those of adults. The liver attains its highest relative size at about 10 of fetal weight at the ninth week of gestation.

Symptoms of newborn jaundice usually develop two days after birth and tend to get better without treatment by the time the baby is about two weeks old. Significant and rapid maturation occurs in the first year of life, with the most rapid elimination of drugs found in school-age children. Liver weight doubles by 12 months of age and increases threefold by 3 years of age.

Jaundice is very common in newborn babies, reaching its peak at about four days of life and gradually disappearing in most babies by the time they are about two weeks old. On rare occasions, an underlying health condition may cause jaundice in the baby’s first 24 hours.

Glycogen is not present in the liver of young foetuses, but it appears at about the 30th week and gradually increases in amount. Hepatic hemopoiesis begins in the foetal liver during the sixth week of gestation, reaches its maximal activity toward the sixth to seventh month, and then regresses. Histologically, the liver is complete by the eighth to ninth month of gestation.

The foetal umbilical portal venous system forms between the fourth and 10th week and is derived from two distinct embryologic precursors—vitelline veins originating from the yolk sac and the umbilical veins originating from the placenta. These veins are largely incorporated into the developing liver as hepatic sinusoids, with some parts of caudal and cranial vitelline segments persisting as separate structures. The ductus venosus allows placental blood to pass from the single left umbilical vein through the portal and then into the IVC and the right atrium.
